Referrals

Requests for additional information or referrals may be made by contacting Amy Fager at (509) 328-7041 during regular business hours (8:00am - 4:00pm Pacific Standard Time, Monday through Friday). 

 

Excelsior contracts with the following entities:

  • Washington Division of Child and Family Services for residential placement, treatment foster care placement, and family preservation services.

  • Spokane County Community Services (Regional Service Network) for mental health residential services, day treatment services and respite services.

  • Spokane Mental Health for emergency respite services.

  • Spokane County Community Services for out-patient chemical dependency services.

  • Montana State Division of Children's Services for residential care placement.

The admissions decision is the responsibility of the Admissions Coordinator and Admission Committee. Their decision is based on input from medical, psychiatric, psychological, and social services as well as other referral sources and the agency's own assessment.  The following list is a guide of information helpful to us in assessing a referral, and identifying treatment concerns.

  • Reason for the request for services.
  • A social history including family background, early child development and a family constellation.
  • A medical history.
  • A history of agency involvement, counseling interventions, and prior placements.
  • Any psychological evaluations or testing.
  • Any school assessments.
  • A copy of any court orders.
  • A Social Security number and copy of the birth certificate.
  • A copy of an up-to-date immunization record.
  • A recent photograph.
  • A pre-admission and/or family interview is also sometimes helpful.

As soon as all pertinent data is received, a decision will be made regarding acceptability for placement and when a possible placement may be available.

While most adolescents are placed through state social service agencies, we will accept private placements.  The cost for a private placement is based on the rate established by our contract with the Washington State Department of Social and Health  services.  These rates include room and board, treatment, and educational costs.  The rates do not include medical or transportation costs.
